Für ökonomische Evaluationen medizinischer Leistungen steht ein etabliertes Methodenspektrum zur Verfügung. Ziel der Arbeit ist, anhand ausgewählter Aspekte herauszuarbeiten, inwieweit diese Methoden für den derzeit viel diskutierten Bereich der Personalisierten Medizin anwendbar sind bzw. welche Besonderheiten dabei auftreten und wie diese adressiert werden können. Für die vorliegende Arbeit wurde eine explorative Literaturrecherche durchgeführt. In Abgrenzung zur herkömmlichen Medizin kann je nach Blickwinkel die Personalisierte Medizin entweder hinsichtlich der physiologischen Unterschiede oder hinsichtlich der individuellen Präferenzen der Beteiligten betrachtet werden. Das Konzept des „Expected Value of Individualized Care“ bietet einen den Informationswertanalysen sehr ähnlichen Ansatz, um den potenziellen Wert eines Einbezugs individueller Präferenzen in die Versorgung zu quantifizieren. Welfarismus und Extra-Welfarismus bieten unterschiedliche Lösungsansätze, wie Nutzen Biomarker-basierter Information im Sinne von „Empowerment“ in Evaluationen einbezogen werden können. Generell sind die Methoden der gesundheitsökonomischen Evaluation auch auf den Bereich der Personalisierten Medizin anwendbar. Die Einführung der Fallpauschalen-basierten Finanzierung der Krankenhäuser ist ein erneuter Versuch, die Kosten in diesem Bereich zu reduzieren. Folge dieses Anreizsystems ist u. a. eine deutliche Verkürzung der Verweildauer der Patienten im Krankenhaus. Das wiederum führt zu durchschnittlich kränkeren Patienten und einer deutlichen Arbeitsverdichtung für alle. Vor diesem Hintergrund wird anhand empirischer Daten die Situation der Pflege mit drastischen Einschränkungen beschrieben und es werden Hypothesen zur Begründung aufgestellt. Daran knüpft sich die Frage, welche Art der Versorgung die Patienten mit kürzer (...) werdender Verweildauer und immer häufiger auftretenden chronischen Krankheiten neben der ärztlichen Therapie benötigen. Dies baut auf der Theorie der Aufgaben zur Krankheitsbewältigung auf, die Patienten befähigen sollen, mit Krankheiten in ihrem eigenen Sinne umgehen zu können. Die Kernaufgaben der Pflege werden hier mit dem Ziel der forcierten Förderung der Entlassungsfähigkeit verbunden mit einem Empowerment der Patienten verknüpft. Organisatorisch bietet sich als Lösung der Einsatz von Primary Nurses an. Die derzeitige Situation in der Pflege widerspricht den genannten Sollvorstellungen diametral. Im Sinne einer sicheren und effektiven Versorgung von Patienten im Krankenhaus muss der Pflegedienst inhaltlich völlig anders aufgestellt und mit weiteren Kompetenzen ausgestattet werden. (shrink)

In der Neonatologie—wie auch in anderen Bereichen der Intensivmedizin—müssen Ärzte und Pflegende häufig die ethisch schwierige Entscheidung treffen, ob die Therapie eines schwer kranken Patienten intensiviert oder eingeschränkt werden soll. In der Literatur existieren verschiedene Instrumente zur Entscheidungsfindung, die, angewandt im Rahmen ethischer Konsile, die Nachvollziehbarkeit einer ausgesprochenen Empfehlung gewährleisten sollen. Zwei dieser verfügbaren Modelle („Schema zur ethischen Urteilsbildung nach dem Muster der aristotelischen Ethik“, Honnefelder 1994 und die „Nimwegener Methode für ethische Fallbesprechung“, Gordijn 1998) wurden anhand klinischer Fälle auf (...) ihre Anwendbarkeit in der Neonatologie hin untersucht. Unsere Studie zeigt, dass die Verwendung eines Entscheidungshilfeinstruments zwar grundsätzlich sinnvoll ist, deckt aber gleichzeitig den Konkretisierungsbedarf der oben genannten Modelle beim Einsatz in der Neonatologie auf. Der daraufhin entwickelte „Kölner Arbeitsbogen zur ethischen Entscheidungsfindung in der Neonatologie“ stellt ein auf die speziellen Bedürfnisse der Neonatologie ausgerichtetes Schema dar und könnte damit eine Lücke für den rasant expandierenden Bereich der Früh- und Neugeborenenmedizin schließen. Im Zuge des „empirical turn“ der Medizin- und Bioethik ist von verschiedenen Autoren in den vergangenen Jahren die Idee einer „evidenzbasierten Ethik“ diskutiert worden. Die Analogie zwischen evidenzbasierter Medizin und „evidenzbasierter Ethik“ soll in diesem Beitrag kritisch diskutiert und dabei gezeigt werden, dass der Ausdruck „evidenzbasierte Ethik“ irreführend ist. Zentraler Ausgangspunkt der Kritik ist die unterschiedliche Bedeutung, die empirische Informationen für das medizinisch-klinische Urteil zum einen und das ethische Urteil in der Medizin zum anderen haben. Im medizinisch-klinischen Urteil können mit (...) Hilfe empirischer Informationen hypothetische Handlungsnormen generiert werden, welche auf den der Medizin inhärenten Zwecksetzungen basieren. Bei Fragen der Ethik in der Medizin hingegen sind die Handlungsziele selbst Gegenstand des Urteils; diese aber können allein aus empirischem Wissen heraus nicht zureichend bestimmt werden. Die Diskussion um die Möglichkeit einer „evidenzbasierten Ethik“ eröffnet weiterhin aufschlussreiche Perspektiven in Bezug auf empirische Forschung und den Umgang mit empirischen Informationen in der Angewandten Ethik und das Aufgabenfeld der Medizinethik. Das Paradigma einer „personalisierten Medizin“ in der klinischen Forschung und Praxis wirft verschiedene Fragen nach Notwendigkeit, Erwartung, Chancen und Risiken auf. In einer laufenden empirisch-ethischen Studie untersuchen wir klinische Forscher- und Patientenperspektiven hinsichtlich des zukünftigen Einsatzes „personalisierter Medizin“ beim Rektumkarzinom. Ziel der Studie ist es, mittels Interviews mit Ärzten/Forschern (n = 19) und Patienten (n = 28) und teilnehmender Beobachtung bei Arzt-Patient-Gesprächen (n = 50) ethisch relevante Aspekte der Erforschung und Behandlung im Kontext „personalisierter Medizin“ zu explorieren. Die Analyse von (...) Unterschieden und Gemeinsamkeiten zwischen den Gruppierungen dient der Detektion von Konfliktfeldern in der klinischen Praxis. Es zeigt sich, dass Patienten und Ärzte unterschiedliche Annahmen von „personalisierter Medizin“ haben. This article concerns a new constraint on the interaction of quantifier phrases and epistemic modals. It is argued that QPs cannot bind their traces across an epistemic modal, though it is shown that scoping mechanisms of a differentnature are permitted to cross epistemic modals. The nature and source of this constraint are investigated.

This article describes a process of developing, implementing and evaluating a clinical ethics support service intervention with the goal of building up a context-sensitive structure of minimal clinical-ethics in an oncology department without prior clinical ethics structure. Scholars from different disciplines have called for an improvement in the evaluation of clinical ethics support services for different reasons over several decades. However, while a lot has been said about the concepts and methodological challenges of evaluating CESS up to the present time, (...) relatively few empirical studies have been carried out. The aim of this article is twofold. On the one hand, it describes a process of development, modifying and evaluating a CESS intervention as part of the ETHICO research project, using the approach of qualitative-formative evaluation. On the other hand, it provides a methodological analysis which specifies the contribution of qualitative empirical methods to the evaluation of CESS. We conclude with a consideration of the strengths and limitations of qualitative evaluation research with regards to the evaluation and development of context sensitive CESS. We further discuss our own approach in contrast to rather traditional consult or committee models. (shrink)

Die demografische Entwicklung und der medizinische Fortschritt werden die Problematik der Ressourcenknappheit im deutschen Gesundheitswesen in Zukunft weiter verschärfen. Soll nicht nur kurzfristig akuten Sparzwängen ausgewichen werden, steht – wie in verschiedenen Ländern bereits geschehen – auch Deutschland auf Dauer eine Prioritätensetzung im Gesundheitswesen bevor. Diese sollte in möglichst transparenter Weise nach klaren Kriterien erfolgen. Eines der seit einiger Zeit häufig öffentlich zitierten Kriterien der Verteilung von Mitteln in der Gesundheitsversorgung ist die Eigenverantwortung von Patienten. Deren Berücksichtigung in der Allokation (...) von Ressourcen birgt allerdings zahlreiche Probleme. Der vorliegende Beitrag untersucht die gerechtigkeitstheoretischen Grundlagen und die praktischen Probleme bzw. die möglichen Konsequenzen von Eigenverantwortung als Verteilungskriterium im Gesundheitswesen. Auf der Grundlage eines alternativen gerechtigkeitstheoretischen Vorschlags werden drei Möglichkeiten skizziert, Eigenverantwortung von Patienten im deutschen Gesundheitswesen in gerechter Weise stärker zu betonen. Onora O’Neill hat 1984 den Zusammenhang zwischen grundsätzlichen Bedenken gegenüber dem ethischen Autonomiebegriff und der Kritik an der paternalistisch geprägten medizinethischen Praxis hergestellt, nicht die tatsächliche Einwilligung des konkreten Patienten zu berücksichtigen, sondern die angenommene, hypothetische Einwilligung, die ein idealisierter, völlig rationaler Patient geben würde. Im Anschluss an experimentalpsychologische Studien zur subliminalen Wahrnehmung, zu Volition und Handlungskontrolle erfahren kompatibilistische Theorien menschlicher Freiheit innerhalb der theoretischen Philosophie neue Popularität. Eine Handlung ist demnach frei, wenn sie das Resultat bestimmter Fähigkeiten einer Person (...) ist („Kompetenztheorie“). Der Artikel setzt sich mit zwei dieser Theorien, Henrik Walters Konzept „natürlicher Freiheit“ und Michael Pauens „Minimalkonzeption personaler Freiheit“ näher auseinander—und zwar vorderhand im Hinblick auf den Entwurf eines nichtidealisierten ethischen Autonomiekonzeptes. Als solches ist es an der aktualen bzw. faktischen Fähigkeit einer Person ausgerichtet, rekurrierend auf die spezifische Determiniertheit autonomer Entscheidungen und Handlungen sowie auf ein relativiertes Konzept menschlicher Vernunft. Pilgrimage and tourism can be related to each other, especially religious tourism. It can be said that pilgrimage is considered an early form of religious tourism due to the fact that these two share similar aspects. The relationship of pilgrimage and tourism with the emphasis on the case of stūpa worship is illustrated in this paper. Stūpa worship is regarded to be an early form of both the pilgrimage and tourism of Tai. The ‘Tai’ in this context refers to those (...) who share Lanna cultural features such as Tai Lanna, Tai Lue, Tai Yai, Tai Khuen, and Laos. (shrink)

In this research, I explored the ways to purify and elevate the soul through various factors, most notably the impact of trials in preserving and elevating the soul. Just as education and moral refinement are crucial for disciplining, thriving, purifying, and reforming the soul, trials also play a significant role in preserving the soul, protecting it from misguidance, and reforming it. Trials contribute to the soul’s ascension in the ranks of servitude to Allah, acceptance of Allah’s decree, and submission to (...) His will. Trials can sometimes serve as expiation for sins, concealing flaws and defects, and at other times they elevate one in the paths of success and righteousness for both individuals and society alike. To understand your true self in your present and past before your burial, observe yourself during times of trials, whether in hardship or ease, in deprivation or abundance. It is in these moments that your true nature, substance, and essence are revealed. Ibn al-Jawzi said: "You will know when the dust clears... whether you are riding a horse or a donkey." It is a well-established fact in religion that the purpose of human existence on this earth is for worship and obedience, and this purpose is achieved through trials. Allah said: "And I did not create the jinn and mankind except to worship Me," meaning: according to the natural disposition upon which Allah created people, whether in happiness or misery. In a lengthy hadith, the Prophet said: "... and prayer is light, charity is proof, patience is illumination, and the Quran is evidence for or against you. Every person starts their day as a vendor of their soul, either freeing it or bringing about its ruin." The only way to live with stability and positivity is through patience in the face of all types of trials. Trials are a part of life, as Allah said: "O mankind, indeed you are laboring toward your Lord with [great] exertion and will meet it," whether this exertion is good or bad, you will encounter it on the Day of Judgment. Every person bears the consequences of their will, choices, and efforts. No one is exempt from this test, hence Allah said: "and will meet it." By this approach, an individual’s character is built and their path clarified. Therefore, it is necessary to understand the concept of trials and tribulations, the difference between these terms, and their outcomes on the purity of the soul and society. This understanding should be utilized for the benefit and development of the community. It is also crucial to understand individual freedom and its limits and to study the philosophy of good and evil, historically referred to as "the creation of actions," to determine responsibility for any actions taken by individuals and to record their movements in light of the reality of the Muslim community and its moderation in all matters. Patience with all types and forms of trials and enduring their consequences in this life and the hereafter is obligatory. I hope I have succeeded in addressing all these points, and our final prayer is that praise be to Allah, the Lord of all worlds. (shrink)

Analytic study of the Advaita philosophy of Śaṅkarācārya and its schools.

In 1987 Uppingham School celebrates the centenary of the death of its best known headmaster, Edward Thring. When Thring arrived at the Rutland market town in 1853 he inherited a small country grammar school of purely local renown, but by the time of his death in 1887 Uppingham was a thriving public school of national reputation. As the school flourished, so too did the headmaster's standing, and in the decade before his death he was a much consulted authority on educational (...) matters. Thring's contributions to educational theory and practice are numerous, but it is in the field of moral education that his influence has lasted longest. This paper examines that contribution. Dialogue is one of the fundamental pillars of social life and an essential means of communication. It enables individuals to express their needs, desires, feelings, positions, problems, and methods for managing various aspects of their lives. Furthermore, dialogue fosters the development of ideas and experiences, facilitating expression, creativity, and participation in a civilized society. Through dialogue, communication and interaction with others occur. This study aims to explore effective methods for teaching the Arabic language through dialogue and its application to non-native (...) speakers at the primary level at Mara University of Technology. Specifically, it will investigate whether dialogue aids students in acquiring vocabulary. Vocabulary is a crucial component of language, alongside sounds and structures. Teaching vocabulary is an essential part of foreign language programs, including Arabic. However, despite its significance, the learning and teaching of Arabic vocabulary to non-native learners face several challenges, contributing to their weaknesses in this area. Notes are statistically significant differences between two groups: those "using dialogue" and those "not using it" in learning Arabic vocabulary. (shrink)

Liberal nationalists have been trying to argue that a suitably sanitized version of nationalism—namely, one that respects and embodies liberal values—is not only morally defensible, but also of great moral value, especially on grounds liberals should find very appealing. Although there are plausible aspects to the idea and some compelling arguments are offered in defense of this position, one area still proves to be a point of considerable vulnerability for this project and that is the issue of what, according to (...) the liberal nationalists, we owe both members of our nation, our co-nationals, and what we owe those who are not members of our nation. It is here that we see the project still has some distance to go if a version of liberal nationalism is, indeed, to be morally defensible. In this paper I examine leading liberal nationalist accounts of our obligations to co-nationals and non-nationals. I argue that liberal nationalists have not yet given us an adequate account of our obligations to non-nationals for a number of reasons. For instance, on the issue of the priority we may give co-nationals' interests over non-nationals', the theorists' view show significant tension, they seem to be confused about what their positions entail, the views are unhelpful, ad hoc, or the positions are quite unclear. Liberal nationalists also have a misleading impression that their positions better capture the relation between personal identity and duty, but this turns out to be false. Other defects with their specific projects are highlighted. I go on to offer a more promising method for determining our obligations to non-nationals. Rather than this alternative precluding any scope for nationalism, it actually makes clearer to us how there might be some defensible space for nationalism once our obligations to put in place appropriate institutions and sets of rules have been fulfilled. The city of Samarra is one of the major cities of Iraq that Yaqout al-Hamwi described in the dictionary of countries by saying (it became the greatest country of God) and Al-Qazwini told about it in the monuments of the country (a great city located on the outskirts of the east of the Tigris between Baghdad and Tikrit) built by the Abbasid Caliph Al -Mu'tasim in God in 221 AH/ 835 M, and it is originally a monastery for the Christians, (...) who bought it for 5,000 dinars, and took it as a capital for its succession, and reached its prosperity and its glory is the days of the caliph Al -Mutawakkil, 247 AH / 861 AD / 892. However, after 58 years, it was left as the capital of the Abbasids when the Abbasid Caliph, Al-Mutamid Allah, abandoned it in the year 279 AH / 892 AD, and took Baghdad, the old capital, as his headquarters, which maintained its position as the capital of the Arab Islamic state until the fall of the Abbasid Caliphate at the hands of the Mongols in the year 656 AH / 1258 AD.Al-Mu'tasim and the caliphs and princes who came after him built palaces, houses, mosques, and other things in Samarra. Samarra was famous for its Great Mosque, distinguished by its high, twisted minaret. It also contained the shrine of the two imams, Ali al-Hadi 254 AH/868 AD and Hasan al-Askari 260 AH/873 AD. A critical study of the author's work is included, along with an examination of King Abdulaziz's character, including his power and leadership, his intelligence and fairness, his compassion and tact, his success to develop the country, and his interactions with his family.

Film production is the process of creating a film, involving stages such as progress, pre-production, invention, post-production, and sharing. It encompasses planning, scripting, shooting, editing, and finalizing the film for public viewing. The process of imagination drives innovation in scripting, directing, and cinematography, resulting in unique, engaging experiences that captivate audiences with their emotive stories. The principle of this investigation is to assess how psychological factors add to enhancing imagination in film production, focusing on their impact on creative processes and (...) outcomes. A study surveyed 680 students from college film program students to analyze their creative personality and imaginative capabilities using structured questionnaires. Key psychological constructs included Creative Self-Efficacy, Emotional Resilience, Cognitive Flexibility, and Intrinsic Motivation. The study investigated the impact of psychological factors on college cinema students' creative imagination in filmmaking, demonstrating high validity and reliability with Factor Loadings exceeding 0.76, Composite Reliability higher than 0.88, and Average Variance Extracted (AVE) range as of 0.59 to 0.67. Discriminant validity was confirmed as every construct AVE was better than its square correlations by other constructs. Psychological factors significantly influence film production imagination, fostering creative thinking and emotional depth. Understanding and harnessing these factors enhances storytelling, character development, and audience engagement, ultimately enriching the cinematic experience. (shrink)

The aim of this research is to extract the Shariah principles from the verses in which the Qur'an addressed the Children of Israel with the phrase: "O Children of Israel." Although the Qur'anic discourse was originally directed at the Children of Israel, but it also addresses Muslims indirectly, as its mention in the Holy Qur'an is for reflection and admonition, as well as for deriving legal principles and rulings. Therefore, the objective of this research is to enlighten Muslims about the (...) Shariah principles derived from the Qur'anic discourse directed at the Children of Israel, to benefit them in their practical lives. The main objective of this study is to formulate a proposed vision to achieve competitive advantage for sustainable urban and human development programs in charitable societies, by determining the reality of competitive advantage, identifying the difficulties facing achieving competitive advantage for sustainable urban and human development programs in charitable societies, and identifying the most important proposals to achieve competitive advantage for those. Programs, and this study belongs to the style of analytical descriptive studies, as the study population is members of (...) charitable societies working in the field of sustainable human urban development in the Riyadh region. The study sample consisted of (65) members, who were selected using a simple random sampling method, and these societies were identified in (the Association Al-Nahda, Al-Wafa Women’s Charitable Society, Al-Ghat Charitable Society, Dharma Charitable Society). The results of the study confirmed that the reality of the competitive advantage of sustainable urban and human development programs in charitable organizations may be represented in: providing the highest possible quality, relying on modern technology in its design, touching reality and spreading the needs of the population, focusing on social innovation in its design, providing it at the lowest possible cost, Community residents participate in its design, community residents participate in its evaluation, and the difficulties facing achieving competitive advantage for sustainable urban and human development programs in charitable organizations have been represented in: the small number of specialized experts in the association, weak resources and technological capabilities, and a weak culture of teamwork among the association’s employees. The weak confidence of the community residents in those in charge of the association, the weakness of the enablers of institutional excellence in the association, and that the proposals to achieve a competitive advantage for sustainable urban and human development programs in charitable societies were represented in: overcoming the organizational conflict between the employees of the association, developing the enablers of institutional excellence in the association, developing resources and capabilities. Technology, developing the comprehensive quality system in managing the association, relying on strategic planning in the association, increasing the confidence of community residents in those in charge of the association. The present study focuses on Using the Technology Acceptance Model (TAM) to study the relationship between teachers’ behavioral intention to use a data projector and some other variables and highlighting the extent to which the TAM is applicable. A random sample consisting of 6480 teachers was randomly selected for the study. For data collection, the author used a questionnaire, which was derived from relevant literature. The employment of Data Show Projectors in teaching Mathematics is influenced by a variety of factors, (...) which chiefly include what is known as computer anxiety (abbreviated as CA). The study shows that CA has a negative effect on PE. Moreover, results of the study also show that there is a positive correlation between actual use (abbreviated as AU) and intention to use (abbreviated as IU). However, results show that perceived usefulness (abbreviated as PU) is not significantly correlated with IU. This means that teachers are more interested in how easy the use of projectors is to them, as opposed to the extent to which it is useful in their work. The study proposes an expansion of the TAM model that takes into consideration the contexts of AU, IU, PU, PE, and CA. Findings show that the proposed TAM model expansion shows positive correlations with AU, IU, PU, PE, and CA. The expanded model shows success in achieving the research objectives. The present study is a contribution to the research efforts for supporting the adoption of projectors in mathematics classes. (shrink)

In recent decades, implementing a code of conduct (CoC) as part of an organization's CSR infrastructure has become a sine qua non for gaining trust and fostering credibility. Despite numerous studies aimed at identifying cultural differences in the content of CoCs, little is known about what causes those differences and how they relate to an organization's communicative endeavor to gain trust and strengthen its legitimacy. In response, this article examines potential cultural differences in the public availability, design, and content of (...) CoCs of corporations headquartered in countries in the Confucian Asian versus Anglo cultural clusters from the perspective of strategic communication. Drawing on the concepts of individualist versus collectivist culture and low- versus high-context communication, the findings reveal significant differences, including that Anglo-based companies more often make their CoCs publicly available and, in turn, significantly more comprehensive than Confucian Asian codes. Furthermore, compared with Anglo CoCs, significantly fewer CoCs of companies headquartered in Confucian Asia address the importance of moral values in daily business practices and sensitive issues such as prohibited behavior, whistleblowing, and sanctions following code violations. Those findings indicate significant institutional and cultural differences in companies' communication about ethical principles and corresponding conduct and suggest that, across cultures, CoCs differ in their content and are not accorded equal relevance as a means to foster legitimacy via CSR communication.
